This letter is to inform you that all students entering the 12th grade will need two doses of Meningococcal (MCV4) vaccine prior to entry. The Logan County Health District will be holding a special clinic on Tuesday May 24th from 4:00pm – 6:00pm, please see flier below. The Logan County Health district also holds immunization clinics every Thursday from 9:00am – 11:00am, and the first Thursday of each month from 2:00pm -6:00pm. You may call (937) 651-6186 for an appointment. Please make sure that these are completed PRIOR to The First Day of School, August 17, 2016 and a copy is on file with your student’s school.
