BHS Common Form

BHS COMMON FORM APPLICATION. The following 20 scholarships use the BHS Common Form Application. The BHS Common Form is due in the Guidance Office no later than April 5.

 

Note:  If the scholarship is part of the BHS Common Form you will only need to complete that form one time with one transcript, although some scholarships may require additional attachments

 

  1. BELLEFONTAINE CITY SCHOOLS BOARD OF EDUCATION SCHOLARSHIPS.  These scholarships, which are awarded by the Bellefontaine City Schools Board of Education and are valued at up to $1,000 each, are based on academic excellence and school and community involvement. 

 

  1. BELLEFONTAINE EDUCATION ASSOCIATION SCHOLARSHIPS.  These scholarships are valued at $1,000 each.  Applicants must be pursuing a career in education or an education-related field, such as guidance counseling or school psychology and/or one or more of their parents must be a current or retired member of the Bellefontaine Education Association (BEA). 

 

  1. BELLEFONTAINE EXCHANGE CLUB SCHOLARSHIP.  Two $1,000 scholarships will be awarded to students with at least a 2.5 GPA who will be attending either a two or four-year school.  Need and community involvement are considered, and the student should note whether he/she graduated from the Exchange Club’s Safety Town Program. 

 

  1. BELLEFONTAINE HIGH SCHOOL ACADEMIC BOOSTER SCHOLARSHIP.  Up to two $1,000 scholarships will be awarded to BHS students attending a four-year university.  Students must have a minimum of a 3.0 GPA, have taken rigorous high school courses, and attach a type response (250 words or less, typed, double-spaced, size 12 Times New Roman font) to the following prompt.  Describe some tasks that you have accomplished over the past two years that have no connection to academic studies. (Names of applicants will be omitted during the selection process.)

 

  1. BELLEFONTAINE HIGH SCHOOL ALUMNI & FRIENDS SCHOLARSHIP.  A $500 scholarships will be awarded by a committee composed of BHS alumni and/or supporters of BHS. Applicants must be graduating seniors who plan to attend a two or four-year college or university and who have at least a 3.0 GPA.

 

  1. BELLEFONTAINE HIGH SCHOOL NATIONAL HONOR SOCIETY SCHOLARSHIPS.  Two $250 scholarships will be awarded to BHS seniors who are currently members of the National Honor Society.

 

  1. BELLEFONTAINE HIGH SCHOOL KEY CLUB SCHOLARSHIP.  Scholarship value ranges between $200-$500.  Applicants must be attending a 2 or 4-year college.  Selection criteria will be based heavily on service performed.  Applicants are required to attach a listing of service activities performed over the last 4 years.  Membership of Key Club is preferred, but not required.

 

  1. GENEVIEV FROMAN SCHOLARSHIP.  This scholarship is valued at approximately $1,000 and will be given to a student attending a two- or four-year school to assist with the first year.  It is awarded by the trustees of the Stieg-Froman estate. 

 

  1. DON HARRIS MEMORIAL SCHOLARSHIP.  This scholarship is valued at about $500 and is awarded to one BHS student who plans to major in Music Education at an accredited college or university. Additional scholarships may be awarded to qualified applicants if funds are available. This scholarship is awarded by a committee of the BHS Music and Friends Association and was established in memory of Donald Harris, who had a great interest in the schools and in the music program.

 

  1. HESS COLLINS FAMILY SCHOLARSHIP. This $1,000 scholarship is in honor of James P. Hess, founder of Hess Lumber, and his wife Ann E. Hess, a Bellefontaine artist. This scholarship will be awarded to a graduating BHS senior planning to attend a 4-year college to study the arts. Students must have a GPA of 3.0 or better and intend to pursue a degree in one of the arts - performing arts, visual arts, dramatic arts, music, film-making, graphic design, fashion design or a related field. Scholarship will be awarded based on grades, need and artistic talent. A recommendation from a teacher familiar with the student's artistic talent is required.

 

  1. LITTLE CHIEFS FOOTBALL SCHOLARSHIP. These scholarships will be awarded to one former Little Chiefs football player valued at $1,000 and one former Little Chiefs cheerleader valued at $500.  Both scholarships will be awarded to assist students in furthering their education. 

 

  1. OHIO ASSOCIATION OF PUBLIC SCHOOL EMPLOYEES SCHOLARSHIP.  These scholarships are given to students who desire to attend either a two- or four-year school.  Scholarships will be given to a child of a classified employee with Bellefontaine City Schools and will be awarded by a special committee of the OAPSE.

 

  1. PEPSI LEADERSHIP SCHOLARSHIP.  Two scholarships valued at $1,250 each will be awarded to BHS students who have demonstrated leadership and who are planning on attending a two- or four-year college.  They will be awarded by a committee from the Bellefontaine City Schools. 

 

  1. RYAN D. REYNOLDS MEMORIAL SCHOLARSHIP.  Ryan was a 1998 BHS graduate who tragically lost his life just prior to leaving for college.  In remembrance of Ryan’s smile, enjoyment for the game of golf, and passion for life, an annual scholarship has been established.  This $500 scholarship will be awarded by the Ryan D. Reynolds Fund Trustees to a BHS student attending a two or four-year college. 

 

  1. DR. EARL SLOAN RENEWABLE BUSINESS SCHOLARSHIP. This $1,000 renewable scholarship will be awarded to a BHS student planning to attend a post-high school institution and major in a business-related field.  It will be awarded by a special committee of the Sloan Foundation.  Two letters of recommendation are required.

 

  1. SONS OF AMERICAN LEGION POST 173 SCHOLARSHIPS. These scholarships are valued at $500 each and are based on grades and need.

 

  1. JAMES STOLLY MEMORIAL SCHOLARSHIP.  This scholarship is valued at $1,000 and was established by the A.E. Knowlton Trust in memory of a longtime Knowlton Construction employee and supporter of Bellefontaine City Schools. The scholarship is awarded by a Bellefontaine City Schools committee. 

 

  1. DEWEY VANHOOSE III MEMORIAL SCHOLARSHIP.  This is a $500 scholarship that will be awarded to an applicant attending Ohio University.

 

  1. DUTCH WILDE MEMORIAL/BHS ATHLETIC BOOSTERS’ SCHOLARSHIPS. These scholarships are worth $1,000, and are awarded to one male and one female student-athlete to assist with either a two or four-year school.  They are awarded by a special committee of the BHS Boosters to a student-athlete or cheerleader. 

 

  1. RONALD H. WISSE MEMORIAL SCHOLARSHIP. This scholarship is valued at about $500 and will be given to a student-athlete to assist during the first year of either a two or four-year school.
